- Wrong drug was prescribed by doctor

- Wrong medication was given to the patient by pharmacist or nurse
- Pharmacist read prescription incorrectly.
- Drug was given where there was a previous allergic reaction. (Sometimes doctors don’t check, and sometimes patients don’t tell.)
- Adverse reaction occurs where no previous allergy was known.
- Drug was given to wrong patient (primarily in hospitals).
- Wrong dose or wrong mixture was prepared.
- Drug was given that interacts with another medication used by patient.
- Wrong drug or dose was taken by patient.
